SPHS AWARD OF EXCELLENCE NOMINATION FORM

Nominations for the SPHS Award of Excellence are to be made based upon the Nominee’s contributions to South Philadelphia High School and/or the South Philadelphia Community. The categories for this award are South Phila High School Executive Board Member, former SPHS teacher and/or administrator, and someone who has contributed to the South Philadelphia Community. The Award of Excellence is awarded at a luncheon in the fall of each year. No posthumous nominations please.
